Proposed relocation of Diabetes Eye Screening Service - short survey

We are considering a proposal to move your diabetes eye screening appointments from Croydon University Hospital to Broad Green Health Centre. 1-13 Lodge Rd, Croydon CR0 2PD (Google maps link - https://goo.gl/maps/UTWCcGwNRVUhN3266). 

The service will be provided by the same staff and nothing else will change. 

If this goes ahead, your appointment letter will confirm the change of venue to Broad Green Health Centre and will include information on how to get there.

Please complete this survey if you usually attend Croydon University Hospital for your regular diabetes eye checks, or if you are a carer or parent / guardian for someone with diabetes who usually attends their checks at Croydon University Hospital.  If you are responding as a parent / carer for someone with diabetes, please answer from their point of view.
